Attached is the Genesis Vicariate Outreach Survey to be filled out by all parishes in the vicariate.  I distributed the survey at the February 25, 2008 VPC meeting.  Please see below for different options for completing and returning the survey.  All hardcopies should be returned to me, Jim Kiefer at the next VPC Meeting, April, 14, 2008 and electronic copies can automatically be sent back to Megan LaMont for input.   

Options for completing the survey: 

1)      Complete the form previously distributed and give it to me

2)      Print and complete the attached form and give to me

3)      Electronically complete the attached form and choose the ‘Submit by Email’ option in   the upper right hand corner

There may be more than one person from each parish who is receiving this survey.  Please take a look at the attached roster and communication list and see who else may be receiving this.  Please check with the pastor to be sure you are the one to submit it.  In addition, if you have updated contact information please send to Monica Archibald - monica@instantassistant.com.
